Replacement Coverage: What is a "Replacement?" A replacement occurs when the applicant cancels, lapses or surrenders an existing policy or certificate of coverage to purchase a new contract with an existing or new insurance company. It can also occur if the applicant uses the cash value from an existing policy or certificate to pay for new coverage.*
Unless you are canceling, lapsing, or surrendering your current coverage, or using cash value from an existing life insurance certificate of coverage to purchase new coverage, you should answer NO to this question.

This notice is to advise you of the Illinois Religious Freedom Protection and Civil Union Act (the Act), and its impact on your insurance coverage.
The Act creates a legal relationship between two persons of the same or opposite sex who form a civil union pursuant to the Act. A marriage between persons of the same sex, a civil union, or a substantially similar legal relationship other than common law marriage, legally entered into outside of Illinois, will be recognized in Illinois as a civil union.
Parties to a civil union are entitled to the same legal obligations, responsibilities, protections and benefits that are afforded or recognized by the laws of Illinois to spouses.
Effective June 1, 2011, the term 'spouse' in policies, certificates, and applications will be considered to include the parties to a civil union. Policies of insurance that provide coverage for children will include coverage for the children of civil unions.
With respect to your "spouse" or "Domestic Partner" or "Civil Union Partner" the following definitions apply:
The term "spouse" means a legally married man and woman.
The term "Domestic Partner" or "Civil Union Partner" means two people of the same or opposite sex who have entered into a personal relationship or union that is recognized by a state or government without the implicit historical and religious meaning associated with the term "marriage." A "domestic partner" or "civil union partner" does not include:
