BENEFITS AND FEATURES
1. Additional Coverage1: In addition to the $12,000 no cost to you coverage, you can purchase Accidental Death and Dismemberment coverage, up to $250,0002;. With this coverage, you can rest assured knowing you are covered around the clock in the event of accidental death, anytime, anywhere. Plus, there is no medical exam to take, coverage is guaranteed to be issued.
2. Coverage at group rates: You cannot be singled out for a rate increase. Rates change only if they are changed for everyone insured under the Group Master Policy. Your coverage will continue as long as premiums are paid as agreed and the Group Master Policy is in force.
3. $100,000 more for Common Carrier Fatality*: When you purchase additional coverage, your beneficiary will collect $100,000 in addition to your primary benefit for any covered accidental death that occurs while you are a fare-paying passenger on any licensed common carrier, such as an airplane, train, bus or taxi.
4. $50,000 more for At-Work Fatality*: When you purchase additional coverage, your beneficiary will collect $50,000 in addition to your primary benefit for any covered accidental death that occurs while you are performing your regularly scheduled work.
5. $25,000 more for Motor Vehicle Fatality*: When you purchase additional coverage, your beneficiary will collect $25,000 in addition to your primary benefit for any covered accidental death that occurs while you are riding in or driving a private passenger vehicle, or as a result of being struck as a pedestrian.
6. Individual or Family Coverage: You have the option of covering your spouse (age 18-69) for 50% of your Accidental Death and Dismemberment benefit, and your unmarried, dependent children under 19 (21 in LA, DC; 25 in NM, TX, WA) or age 25, if a full-time student, for 10% of your Accidental Death and Dismemberment benefit. In IN, your dependent children under age 26 will be insured regardless of marital or student status. When your spouse reaches age 70, coverage will reduce to 50% of the full benefit amount.
7. Accidental Loss and Dismemberment: If you're in a covered accident, but not fatally injured, benefits will be paid for loss of sight, hearing, speech or limbs, ranging from 10% to 100% of the accidental death benefit you choose. Loss must occur within 365 days of the accident.
8. Premium Waived during Strikes and Lockout: A special benefit designed for active union members which pays your premiums for up to a full year in the event of a union-sanctioned Strike or Lockout as a result of a Labor Dispute lasting 30 or more days.
9. Inflation Guard: Benefits increase automatically. That's right, every two years, on the anniversary of your Certificate, the original amount of additional coverage will automatically increase by 5%, until your coverage has increased a full 25%. As long as your Certificate remains in force, there will be no increase in premium for this extra benefit.
10. Your Right to Review: You have 60 days from the date you receive your Certificate to return it. If you decide you don't want the coverage, all premiums paid will be refunded to you and you'll be under no further obligation. Whatever you decide, your no cost to you insurance benefits remain in force for one year.
1 If you are covered by more than one similar Union Labor Life Certificate or Policy, for which you pay premium, your total face amount of Accidental Death and Dismemberment benefits cannot exceed $300,000.00.
2At age 70, benefits reduce to 50% of the full benefit amount.
*Death must occur within 365 days of the covered accident. Note: Additional benefits for Common Carrier, At-Work and Motor Vehicle fatalities apply to the main insured only. They do not apply to your covered spouse or children. Only one benefit, whichever is the greatest, will be paid for these accidents.
