BENEFIT SUMMARY
1. Benefits paid directly to you in the event of an accident: All covered benefits are paid directly to you or to a doctor, hospital or anyone you choose. There are no restrictions with how you use your benefits. Once paid, benefits are yours to use as you wish.
2. Protection valid 24 hours a day, 365 days a year (on the job and off): This coverage is NOT limited to the workplace. Your Hospital Accident Care coverage is valid on the job, at home, on vacation, anywhere. Some injuries as result of an accident are excluded.
3. Benefits paid in addition to any other coverage or plan: This plan provides benefits in addition to any other health insurance you may already have. Coverage includes:
In-Hospital Benefits
- $200/day for Hospital or $250/day for Intensive Care
- With Family Coverage, if you and your insured spouse are confined at the same time for the same accident, benefits may double while you are both hospitalized
Out-of-Hospital Benefits
- $50 Ambulance (up to 5 trips/year)
- $25/day Physician Follow-Up Visits (up to 5 days/year)
- $50/day Daily Outpatient Facility (up to 5 days/year)

Full benefits up to age 80: You are eligible for full benefits up to age 80. At age 80, your benefit amount will reduce by 50%. The protection is yours for life as long as premiums are paid, and the Group Master Policy remains in force.

Rates will never increase based on your age or health: You will never be singled out for a rate increase because of your age or health. Rates may be changed only if they are changed for all insureds under the Group Master Policy. Your coverage will continue as long as premiums are paid as agreed and the Group Master Policy remains in force.

Strike Waiver of Premium: Offers a special benefit designed for active union members which pays your premiums for up to one year in the event of a union-sanctioned strike or lockout as a result of a Labor Dispute lasting 30 or more days (not available in all states).

All family members covered for one monthly price: You can take coverage for just yourself (union member) for $7.95 per month or for your entire family for $15.95 per month. Family coverage protects the union member, spouse and unmarried dependent children under age 19 or age 25, if they are full time students.
